How to Switch Home Care Providers: A Step-by-Step Guide

1. Evaluate Your Current Care Plan

2. Research New Providers

3. Inform Your Current Provider

Once you’ve decided to switch, let your current provider know. Giving formal notice ensures there are no disruptions in your care during the transition period. Since January 2023, providers are prohibited from charging exit or joining fees, so switching won’t cost you any additional fees. 

4. Contact Your New Provider

You have 56 days from your agreed end date with your current provider to engage with your new provider. If you choose Colbrow Care, we will help you create a personalised care plan and guide you through the necessary paperwork. 

5. Transfer Your Funding

Call My Aged Care on 1800 200 422 to re-activate your referral code and provide it to your new provider. Following changes in September 2021, Services Australia holds most funds in your Home Care Account. Your old provider will finalise your account, and the funds will be available to your new provider within 70 days. 6. Confirm Transition Date and Start Your New Care!

Coordinate the end date with your existing provider and the start date with your new provider. This ensures there are no gaps in your care services. Once everything is in place, you can begin your new care with your chosen provider.
