How does home care work? This is a question we are asked often! Our process begins with you making a call or sending an enquiry through our website. Once we have your initial details, we will schedule a time convenient for you and one of our friendly Case Managers to conduct a Care Assessment and an OH&S Assessment.
In consultation with you and your family or personal representative, your assigned Case Manager will develop your individualised support plan which will detail how in home care works and the supports and services we have matched to your needs.
Your needs are of paramount importance and we therefore follow a person-centered approach when planning your care.
Once your support plan is implemented, you will have a Care Coordinator and care staff assigned to you. Colbrow’s head office team will create rosters for the staff that are going to be directly involved in your care.
We like to get regular feedback from our clients to get an understanding of how they are going with the support being provided and so that we know what we are doing right and what we need to improve on. Your satisfaction is the best indicator that we are accomplishing our goal and we will continue to liaise with you regularly.
