Our process begins with you making a call or sending an enquiry through our website. Once we have your initial details, we will schedule a time convenient for you and one of our friendly Case Managers to conduct a Care Assessment and an OH&S Assessment.
In consultation with you and your family or personal representative, your assigned Case Manager will develop your individualised support plan. Your needs are of paramount importance and we therefore follow a person-centered approach when planning your care.
Once your support plan is implemented, you will have a Care Co-ordinator and care staff assigned to you. Colbrow’s head office team will create rosters for the staff that are going to be directly involved in your care.
We like to get regular feedback from our clients to get an understanding of how they are going with the support being provided and so that we know what we are doing right and what we need to improve on. Your satisfaction is the best indicator that we are accomplishing our goal and we will continue to liaise with you regularly.
A time is scheduled with you and one of Colbrow’s Case Managers to conduct a Care Assessment.
An OH&S Assessment is also conducted.
