Review your current contract for your notice period and give notice to your current provider.

Call My Aged Care on 1800 200 422 to re-activate your referral code and provide it to Colbrow Care. Confirm your end date to the existing provider.

Have a look at your current Home Care Package Agreement for any contractual notice periods with your current provider.

Yes, they are required to provide services to you until the agreed end date.

Since January 2023 providers are not allowed to charge exit or joining fees. The switching process will not attract any additional fees from either party.

You have 56 days from your agreed end date with your current provider to engage Colbrow Care. Failure to meet this timeline will result in your package being withdrawn.

Your Case Manager will provide you with our current price list and Home Care Agreement. We will work with you to prepare an Individualised budget based on your funding, and will never increase fees or add extra costs without your agreement. Typically our fees change during July each year, but we always notify our clients of these in advance, so services can be adjusted, if needed.

Due to changes made in September 2021, most funds are held by Services Australia in your Home Care Account. Your old care provider will finalise your account, and the funds will be available in your Services Australia account , ready for your new care provider within 70 days.

Of course! But we would like to think that a conversation around your motivation for doing this might help re-align any expectations or preferences you feel are not being met. Should this not be successful, you have the same rights to change to a difference provider as you have now.
