Registered Nurse – Clinical Coordinator

We are inviting all registered nurses, with a special interest in Clinical Coordination for our client located in Kensington.

The Role

Coordinate the day-to-day and long-term support needs of client, which are high in complexity (both extensive and intensive) and focusing on maintaining his existing personal independence and enhancing independence where possible.

As the Clinical Coordinator, your role is to minimise the overwhelming impact associated with identifying, organizing, implementing, overseeing and coordinating a broad range of support services that if not for this position, would have to be identified, organized, implemented, overseen and coordinated by our client and his family and/ or household.  This role is carried out in cooperation with the employing agency and the Transport Accident Commission (TAC).

Our client wishes to pursue life’s opportunities as autonomously as possible and requires extensive and intensive support to do so; but does not want this required support to (unnecessarily) disrupt his personal, household or family relationships, nor to stymie his personal, social or vocational pursuits.

Key Responsibilities

  • Coordination, training, and supervision of the nursing and attendant care worker team.
  • Identifying, organising, implementing, overseeing and coordinating other necessary supports needed by reason of our client’s disability.
  • The planning, management, administration and coordination of funded support required to meet our client’s direct-care and day-to-day and longer term support requirements in accordance with Transport Accident Act 1986 entitlements, VCAT/AAT decisions and TAC guidelines (where appropriate).
  • Maintenance of professional standards of practice in accordance with APHRA (Australian Health Practitioner Regulation Agency).
  • Monitoring support requirements to identify where change is needed and/or additional support required.

You will be required for 20 hours per week for your coordination role along with additional direct care shifts.


Registered nurses will have:

  • Bachelor of Nursing
  • Current AHPRA registration

Registered nurses will also have preferred experience in the following:

  • Respiratory/ Ventilator/ Tracheostomy nursing experience
  • Spinal Injury nursing experience
  • Experience in similar Team Leader/Coordinator/Case manager role
  • Community/ Homecare experience

  • Current Police Check and Working with Children’s Check
  • NDIS Worker Screening Check (includes Police Check)
  • Valid working rights in Australia
  • First Aid Certificate (HLTAID011) and CPR (HLTAID009)
  • Mandatory competencies (As required by Colbrow Care)
  • Current Drivers Licence and reliable transport
